1. I will conduct myself and endeavour to ensure others in my company conduct themselves in a proper and reasonable manner and in accordance with Basketball’s Codes of Conduct, a copy of which is available from Basketball Queensland’s website.
2. I agree to be bound by the Southwest Queensland Sport and Recreation Association’s constitution rules and bylaws and the conditions of entry into the basketball facility and to submit myself to the jurisdiction of their Tribunal, and any other disciplinary forum connected with basketball, Constitution can be found: www.supercamp.com/swq
3. I agree to comply with any reasonable direction of any official connected with basketball whilst at the venue or on road trips.
4. I acknowledge that I am aware that there will only be limited times when a qualified first aider will be present.
5. I authorise the club, association, league to obtain medical treatment for me or my child should there be an emergency and it is not possible or reasonably practical for me to do so. I also will accept the cost of any ambulance or medical treatment.
6. I advise that I or my child has the following medical conditions, including allergies and warrant that I am or my child is fit to play basketball and will notify the association should any medical information become available.
*IF YOU DISAGREE WITH ANY OF THE FOLLOWING BELOW PLEASE STATE ON A DECLARATION FORM.
a. I or my child may be photographed, and any photograph used in any newsletter, (whether printed or electronic) for the promotion of basketball.
b. Videos may be taken of me or my child playing or training to be used for family keepsakes by family member of other players, or by or for coaching staff to be used for coaching or scouting purposes.
c. Information about basketball will be sent to me from time to time by mail or email.
d. I will not photograph or video any activity in which I or my child is involved unless I make it known to the coach of team manager, who I am and that I propose to take photographs of video a game.
e. I will be responsible for any damage to property and equipment caused by me or my child and pay for such damage on demand.
f.Parent’s Consent must be completed for all players under 18
While participating in physical activities, each participant is assumed to be voluntarily performing these activities, for which he/she assumes all risk, consequences and potential liability. The Southwest Queensland Sport and Recreation Association Inc and its staff shall not be held responsible for any medical cost by reason of accident, illness, death and other consequences arising or resulting directly from participation in the program. Should a participant become ill or injured, the Southwest Queensland Sport and Recreation Association will transport the player to the local Base Hospital (if able to) or call an Ambulance.
